Failure In Leadership
In other words, while Geren is singling out the retired General Kensinger for a slap on the wrist, considering he admitted to reporters the man deliberately deceived investigators and the press to cover his own ass, and while he admits the Army failed at every level in handling the affair appropriately, he still can’t bring himself to say anyone was really at fault.

Army Major General Antonio M. Taguba's investigation of army regulation violations at Abu Ghraib is a story getting little attention in the main stream media.
Late January 2004 Taguba was given the job of investigating Abu Ghraib because of circumstance: the senior officer of the 800th Military Police Brigade, to which the soldiers in the photographs belonged, was a one-star general; Army regulations required that the head of the inquiry be senior to the commander of the unit being investigated, and Taguba, a two-star general, was available.

Dignitaries from the prime minister down didn’t bother to conceal their contempt for those who impugned their motives in making such a big deal of Kovco’s death.

If you are wondering why this was signed by a Marine, notice that JTF 121 was a Joint Task Force with Army Rangers, Seal Team Six, and JSOC. Those who might not be familiar with the Army regulations should refer to U.S. Army Regulation 670-1:
